What is a 329 Card?
A 329 Card is the Hawaiʻi Department of Health (DOH) medical cannabis registration that allows qualified patients to use medical cannabis for approved conditions. The application and renewal are handled online through the Medical Cannabis Registry.
Who Qualifies?
Adults (18+) with a qualifying condition under Hawaiʻi law; minors may apply with a caregiver.
Common qualifying conditions include:
chronic pain
PTSD
severe nausea
cancer
glaucoma
seizure disorders/epilepsy
HIV/AIDS, cachexia
multiple sclerosis
Crohn’s disease
rheumatoid arthritis
lupus
and other debilitating conditions as approved.

How Our Process Works (intake first)
Start in our Client Portal – Complete the short intake and upload any relevant records. We’ll review and follow up to schedule your initial in-person evaluation. (We confirm all 329 certifications only after intake review.)
Apply with the State – Create your eHawaii.gov account and complete the DOH Medical Cannabis Registry application; pay the state fee online. Hawaii State Department of Health
In-person evaluation – Meet in clinic to confirm eligibility, review risks/benefits and finalize your certification.
Approval & instructions – If qualified, we complete the DOH provider portion and you’ll receive DOH confirmation with access instructions for dispensaries.
Note: After your first in-person certification, annual renewals and routine questions can often be handled by telehealth, as appropriate.
What You’ll Need
eHawaii.gov login (for the DOH registry) and ability to pay the state fee online. Hawaii State Department of Health
Government photo ID and basic demographic info.
Qualifying condition documentation (recent visit notes, imaging, relevant labs when available).
For minors: caregiver details as required by DOH (see DOH guidance).
Visitors & Out-of-State Patients
Hawaiʻi allows out-of-state medical cannabis patients to apply for a temporary registration online (start up to 60 days before your visit; up to two registrations per calendar year). Review DOH eligibility and island dispensary availability, then apply via the DOH portal.
